Dental Office Patient Information:
First visit:
Your initial appointment will consist of a diagnosis and discussion of treatment options. If treatment cannot be done the same day, a second appointment may be needed. Please assist us by providing the following information at the time of your consultation:

Your referral slip
- Recent X-rays (within 6 months), or we also provide X-Ray services.
- A list of medications you are presently taking.
- Dental insurance information and any necessary completed forms.
- Medical history and any medical condition that may be of concern.
- IMPORTANT: A parent or guardian must accompany all new patients under 18.
Scheduling:
Our dental offices are open Monday-Friday from 8 am to 5 pm. If you have pain or an emergency situation, every attempt will be made to see you same day.
Billing/Insurance:
For your convenience we accept Visa, MasterCard and Discover. Payment is due at the time service is rendered unless advance arrangements have been made. Your first bill usually arrives before your insurance has made payment. Please call our office if your statement does not reflect any insurance payment after 4-6 weeks. Any balance remaining after your insurance has paid is your responsibility.
Infection Control:
We always follow procedures by OSHA (Occupational Safety & Health Administration), the EPA (Environmental Protection Agency) and the CDC (Center for Disease Control). These measures include disinfectant hand soap, gloves and face masks, chemical disinfection of countertops and surfaces, sterilization of all reusable equipment, and disposable materials. We use an autoclave (disinfects by steam, heat and pressure). |
